Sam’s club is a chain of retail warehouse clubs, which has conducted customer survey to measure how their members feel about the services and products they have received from Sam’s Club. Limit: Eligible entrant can enter once during sweepstakes period. In addition, their age must be 18 years at the time of entry.

How to enter: There are three ways to get entry in to the sweepstakes.

Purchase: Make purchase through any participating store and get product receipts. After that, visit www.survey.samsclub.com within two (2) weeks from the cash register receipt date and enter the unique code located on the cash register receipt to complete the survey. Upon completion of survey, you will get one entry in to the sweepstakes.

Online: In order to get entry without purchase or submitting survey, visit www.entry.survey.samsclub.com and complete and submit official entry form with all required details like full name, mailing address, phone number and age.

Mail: You can also get entry by sending your personal details like complete name, mailing address, phone number and age on a postcard or a card (no smaller than 3 ½ x 5 inches and no larger than 4 ¼ x 6 inches), to 2016 Q1 Sam's Club Sweepstakes, PO Box 10310, Rochester, NY 14610.
