Purchase Method of Entry: An eligible entrant may visit a participating Darden restaurant location and complete a purchase. Upon making a purchase, the entrant will receive a receipt with a survey code and promotion URL. Entrants may visit the URL at lhtogosurvey.com to complete and submit the LongHorn Steakhouse Survey. The entrant must then complete and submit the registration form by providing the information requested by Sponsor including, but not limited to: complete first and last name, street address (P.O. Boxes are not permitted), city, state/province/territory, ZIP/Postal Code, daytime phone number, date of birth, survey code, and agree to the Official Rules.

Free Mail-in Method of Entry: An entrant may handwrite his/her complete name (no initials), valid e-mail address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY), on a 3½"x5" card and mail it (in an outer mailing envelope) with sufficient postage to: Guest Satisfaction Survey Sweepstakes, PO Box 211, Macedon, NY 14502-0211. Mail-In Entry is worth 1 entry into the LongHorn Steakhouse Survey Sweepstakes.

Scan QR Code: An eligible entrant may visit a participating Darden restaurant location and scan the Promotion QR code provided on the Promotional material. Upon scanning the QR code, the entrant will be prompted to register for the LongHorn Feedback Survey Promotion, provide the survey code, and complete the survey. Upon completing the LongHorn Steakhouse Survey, the entrant will receive 1 entry into the applicable Entry Period.

Email Invitation: An eligible entrant who receives an email invitation may follow the links and instructions in the email to register and complete the LongHorn Steakhouse Guest Satisfaction Survey. Upon completing the survey, the entrant will receive 1 entry into the applicable Entry Period.

LongHorn Gift Card Giveaway Survey Winners Selection:

There will be a total of 60 winners. Per entry period, five winners will be selected randomly from all eligible entries on or about the following drawing dates. The LongHorn Survey Sweepstakes winners will be notified within five days after the drawing.
