At the end of the survey, you may enter the sweepstakes.

Mail-in Entry:
Here you can enter without completing a survey. At first, hand-print your complete name, complete address (no P.O. Boxes) , city, state, zip code, telephone number and e-mail address (if you have one) on a 3" x 5" card.
