How to enter

There are 2 ways to enter:

• Mail-in Entry: In a number #10 envelope insert 1 3×5 card with the following information: handprint name, address, city, state, and zip code, email address, telephone number, and date of birth. On the outside of the envelope, in the upper left of the envelope, handprint your name and address, handprint, and mail to: Double Trouble Shirt Shop, P.O. Box 488, Syosset, New York, 11791-0488.
