Mail-in: On a postcard, write down your name, name, address, telephone number, age, email address and the specific Sweepstakes in which you want to enter and mail it to: 2017 “GUEST SATISFACTION SURVEY” PROMOTION #3-12, 22287 Mulholland Hwy. #396, Calabasas, CA 91302.
