Method #2 AMOE (Alternate Means of Entry) - hand print your name, complete address, city, state, zip code, daytime phone number, and birth date on a 3” x 5” card and mail it to: Golden Corral Survey Sweepstakes Entry, PO Box 728, Social Circle, GA 30025-728.
