Complete the registration form with the requested information, including your name, email address, mobile number, mailing address, and age.

• You must also submit a written essay of 1,500 characters or less explaining why you are an outstanding teacher who goes above and beyond for your students and deserve to win the $1,000 prize.
• In addition, entrants must upload a recent photo for potential publicity purposes and must have permission from the photo’s copyright owner before submission.
