First make a purchase of any food or beverage at a participating World of Beer Bar & Kitchens during the promotion period, after that visit www.tellwob.com, enter the location that you have visited, and the 16-digit survey code from your cash register receipt and complete the survey. Upon completion of the survey, you will get an opportunity to enter the sweepstakes drawing.

Mail-in Entry:

In order to enter, just write down your personal details your name, complete address (including city, state, and zip code), valid email address, age, and daytime and evening phone numbers on a 3” x 5” card or piece of paper and mail it in business-size envelope with first-class postage affixed to World of Beer Survey Sweepstakes, 10910 Sheldon Road, Tampa, FL 33626.
