Upon completion of the survey, you will get an opportunity to enter the sweepstakes drawing.

Mail-in Entry:

In order to enter, just write down your personal details your name, complete address (including city, state, and zip code), valid email address, age, and daytime and evening phone numbers on a 3” x 5” card or piece of paper and mail it in business-size envelope with first-class postage affixed to World of Beer Survey Sweepstakes, 10910 Sheldon Road, Tampa, FL 33626.
