Domino’s Voice of the Customer Survey Sweepstakes Rules:

• You must be at least 18 years of age to be eligible for the Domino’s Survey Sweepstakes.
• Only legal residents of the 50 United States and the District of Columbia may enter the Domino’s Feedback Survey.
• Domino’s employees, affiliates, Merkle Inc. staff, and immediate household members are not eligible for the Domino’s Customer Satisfaction Sweepstakes.
• No purchase or survey completion is required to enter or win the Domino’s Sweepstakes.
• A maximum of one entry is allowed per person per Entry Period, regardless of method.
• A valid email address and contact number are required to participate in the Domino’s Voice of the Customer Survey.
• Prizes awarded in the Domino’s Gift Card Giveaway are non-transferable and cannot be exchanged for cash.
• All taxes related to winning a prize are the sole responsibility of the prize winner.
• Each survey invitation allows only one entry into the sweepstakes.

How to Participate in Domino’s Customer Satisfaction Survey:

There are two ways to enter during the promotion period:

Online Survey Entry: If you receive a Domino’s email invitation to complete the Voice of the Customer Survey, follow the link and submit your honest feedback within 15 days of receiving the invitation. Or visit Domino’s Survey website (dominosfeedback.com) directly and complete the survey. At the end, opt in to enter the sweepstakes. One entry will be submitted into the drawing period that aligns with your submission date.

Mail-In Entry: To enter without completing a survey, handwrite your name, mailing address, email, phone number, and date of birth on paper and mail it in an envelope to:

Be sure to clearly note which Entry Period (1or 2) you are entering on the outside of the envelope.

Dominos Survey Sweepstakes Winner Selection:

Per entry period, 100 winners will be selected randomly on or about following drawing dates: August 14, 2026, September 15, 2026, October 14, 2026, November 9, 2026, December  14, 2026, and January 14, 2027.

Domino’s Customer Feedback Survey Prizes:

A total of 600 winners (100 per Entry Period) will each receive a $25 Domino’s e-gift card.
