After that complete and submit the official entry form with all required details like your name, birthday, email address, daytime phone number, and mailing address.

Mail-in: Write down your personal details like name, address, city, state, zip code, e-mail address, day and evening telephone numbers, and the words “Michaels 2017 Holiday Sweepstakes” on a 3” x 5” card and mail it to Category Marketing Specialist, 8000 Bent Branch Drive, Irving, TX 75063. Entries submitted by mail must be postmarked by November 15, 2017.
