Upon completion of survey, you’ll be automatically entered to win $1,000 in cash.

Limit: You can enter maximum five times per drawing period regardless of method of entry. Entrant must be reached at the age of majority in their province.

Total entry period is separated in four quarterly periods.

How to enter:

There three ways to enter the sweepstakes:

1) Online: During promotion period, visit www.eyewearsurvey.com and enter store number and transaction number from your receipt and complete the eight-question survey. After completing the survey, you have to enter valid email address for which you are the authorized user and confirm your entry by clicking through a link in a confirmation email that will be sent to you. Once you confirmed, you will get one entry in to the quarterly survey sweepstakes.

2) If your email address has been collected at a participating Visionworks store, then you will get an email inviting you to take a survey, click through the link provided invitations.

3) In order to enter without completing the survey, send a postage-paid, 3½"x5½" postcard with your hand printed name, complete address, daytime phone number, date of birth and valid email address to: Visionworks Survey Sweepstakes, c/o The Marketing Agency, PO Box 39149, Fort Lauderdale, FL 33339-9149.

One grand prize winner will be selected randomly within seven day after the end of each quarter period.
