After that upload the photo of your purchase receipt along with your personal details like full name (no initials), complete mailing address (no P.O. Boxes), telephone number, and date of birth and click “Submit”. Photo must be in .JPEG, .JPG, or .GIF format.

Non-purchase method:

In order to enter without making purchase, just write down your personal details like complete name (no initials), valid e-mail address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth on a 3½”x5” card and mail it to Chuck E. Cheese’s Biggest Birthday Ever Sweepstakes, P.O. Box 251328, West Bloomfield, MI 48325.
