How to Enter:

There are two ways to get entry into the promotion:

Online: visit www.UHCStepUp.com and follow on-screen instructions to take the “Step Up for summer” pledge to be healthier and then submit your pledge to receive an entry into the promotion.

Mail-in: Write down your personal information including your complete name, address, phone number, and date of birth on a 3″ x 5″ card and mail it to 2021 Step Up for Sweepstakes and World Record Challenge, 79 Madison Ave Suite 15021, New York, NY 10016.

Winner Selection:

One grand prize, one golf prize, fifty first prizes, and fifty second prize winners will be selected via random drawing on or about September 30, 2021, from all eligible entries.
