Each unique code can be submitted one time only. You will earn 1 Online Entry in the Sweepstakes per unique code. There is a limit of up to 8 Online Entries per person.
• Mail-in method (No Purchase Required): Write your full name, complete mailing address, valid email address, and telephone number on a piece of paper, insert that paper in a #10 business size envelope, affix postage, and mail via USPS First Class mail to: Surfer Boy Pizza Sweepstakes Entry, P.O. Box 157, Palatine, IL 60078-0157. Each Mail-in Entry must be written on a piece of paper and only 1 Mail-in Entry per envelope. Limit up to 8 Mail-in Entries per person.
