Visit Sprouts.com/signup to create a free Sprouts account. After that, receive an email with the Sweepstakes entry form which you must fill out and submit to receive an entry into the promotion.

Visit www.Sprouts.com/signup and fill out the official entry form with all requested details to receive one initial entry.

Mail-in: Just send your handwritten personal details including your name, address, age, and daytime/evening phone numbers, and the word SWEEPS on a plain 3”x 5” piece of paper to Sprouts Healthy Groceries Sweepstakes, 5455 E High Street, Suite 111, Phoenix, AZ 85054.
