You will also be given the option to join the Planet Smoothie eClub, see planetsmoothie.com/eclub/. All Entries must be complete and accurate to be eligible.

To receive 1 entry without signing up for Planet Smoothie’s eClub, on a 3" x 5" piece of paper, hand-print your name, complete address, daytime and evening telephone numbers (area code included), and date of birth, and mail entry in a properly stamped #10 envelope to: Kahala Management, LLC – Kidz Bop Planet Smoothie Giveaway, 9311 E.
