Provide the following: Your full name, phone number, and state of residence, Confirmation that you are 18 years or older, your connection to the nominee (if not yourself), Nominee’s full name, phone number, email, state of residence, and confirmation that they are 18 or older. 

Write 100 words or less explaining why your nominee should win, highlighting their passion for youth sports and any barriers preventing attendance at games.
