Mail-in: Handprint name, complete home address, day and evening phone numbers, email address (if any), age, and special event code provided at the U.S. Bank event on a 3” x 5” card and mail by United States mail in a sealed envelope no larger than 9 ½” x 4 ¼” (a #10 envelope) with correct postage to Compliments of U.S. Bank, U.S. Bank, Attn: Marketing-CN-OHW11M, 425 Walnut, Cincinnati, OH 45202.

U.S. Bank Cash Sweepstakes Winners Selection:

One potential grand prize winner will be selected from all eligible entries on or about March 5, 2021. The winner will be notified via phone or email. He/she must execute and return an affidavit of eligibility, a liability release, and, a Publicity Release in the form provided by the Sponsor in order to claim the prize.
