How to Enter

The entrant must follow the online instruction to complete submission of his/her entry with an e-mail address, full name, postal code for state of residence, gender, and date of birth.
• Mail-in: An eligible entrant may hand write his/her complete name, valid email address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY) on a 3½” x 5” card, and mail it with sufficient postage to: 12 Days of Deep Eddy Sweepstakes, P.O. Box 563, Bardstown, KY 40004.
