• A recent purchase receipt or donation is mandatory
• You must take the survey within 7 days of your purchase
• Coupon code must be used within 90 days
• The survey is available in English and Spanish.
• The coupon is not redeemable for cash
• The coupon cannot be used for new merchandise (red tags) or gift certificate purchases
