Keep the original itemized receipt, take a clear photo or screenshot showing the store, purchase date, and qualifying product, then visit https://www.youtheory.com/pages/sweepstakes, complete the entry form, upload the receipt image, and submit it to receive one (1) entry. Each submitted receipt must be unique.

Free Mail-In Entry: Hand-print your name, complete address, email address, and phone number on a sheet of paper and mail it in an addressed envelope to: The youtheory Wellness Rewards Sweepstakes, PO Box 595, Social Circle, GA 30025-0595. Each Mail-In Entry must be mailed separately. Up to fifty (50) Mail-In Entries per person are permitted.

Bonus Purchase Entries: One additional qualifying product earns two (2) Bonus Purchase Entries; two additional qualifying products earn five (5) Bonus Purchase Entries.
