Just share everything regarding your shopping experience with United Family Supermarkets in customer survey to make it better and better. Upon completion of survey, you will get opportunity to win 1 of 6 $100 gift cards every month.

There are three ways to take a part in to the survey sweepstakes:

Visit retail store: During promotion period, make a purchase from participating retail locations to participate in a confidential online customer survey. Upon completion of the survey, you have to enter your personal contact information like Name, Mailing address, Telephone number, and email to get one sweepstakes entry.

Online: Visit www.theunitedfamily.com/survey and submit online survey with loyal feedback. After completing survey, you have to enter your personal details to get one entry in to the sweepstakes.

Mail-in: You are also able to get entry in to the sweepstakes without submitting online or offline survey. For it, write your complete name, complete address (no P.O. Boxes), city, state, zip code, telephone number and e-mail address on 3” x 5” card and then mail it to UNITED SUPERMARKETS “Survey Sweepstakes”, c/o StellaService, 75 Broad Street, Suite 1010, New York, NY 10004.
