Make a purchase of food or beverage at a participating Chipotle Mexican Grill restaurant and collect the receipt. After that, visit www.chipotlefeedback.com or website printed on your receipt and enter your receipt code to get started. Then, answer all survey questions to complete the survey. Upon completion of the survey, submit the registration form with the required details to receive one entry into the sweepstakes.

Mail-in:

Hand-write your complete name (no initials), valid e-mail address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY) on a 3½"x5" card ("Mail-In Entry") and mail it with sufficient postage to: Chipotle Feedback Customer Satisfaction Survey Sweepstakes, PO Box 251328, West Bloomfield, MI 48325. You will receive one entry into the sweepstakes.
