There are two ways to enter: online and mail

After that, answer questions regarding your most recent experience at the store and provide your personal details: name, address, e-mail address, telephone number and date of birth. You will receive one entry into the sweepstakes drawing.

Mail: Hand print your name and residential address, telephone number, and date of birth on a 3” x 5” card and submit it in a #10 envelope to: Guest Survey Monthly Sweepstakes, P O Box 1411, Tyler, TX 75710.
