In order to enter, just write down your personal details including your full name, complete address, telephone number, email address, and date of birth on a plain 3" x 5" card and mail it to: "Bolla Winter" Sweepstakes, c/o One Line Sports Agency, P.O. Box 610505, Bayside, NY 11361-0505.

QR Code Entry:

Just scan the QR code located on the Sponsor’s printed communication by using your Smartphone to access to sweepstakes URL. After that, complete and submit the entry form with all requested details.

Bolla Winter Sweepstakes Winner Selection:

All potential winners will be selected randomly from all eligible entries on or about April 7, 2021, and notified by telephone or email. They all have to sign and return an Affidavit of Eligibility, Release and Indemnification and a Publicity Release within three business days to claim the prize.
