AutoZone is conducting customer satisfaction survey to get your opinions and suggestions about their products and services. If you have recently visited at any AutoZone store and you want to give suggestion or to make complaint, then you are invited in this survey. AutoZone will get help in improving its product range, customer service and general store quality with the help of your suggestions. As you complete the survey, you will get entered in the sweepstakes to get a chance to win $5,000 per quarter.

Limit: One entry per person per email address during promotion period. Participants must be 21 years old or older at the time of entry.

AutoZone customer satisfaction survey is very simple and takes a few minutes to complete. Visit www.autozonecares.com, select the country and the language from English or Spanish, then enter the reference number that is printed on your sales receipt to start the survey. Answer the survey questions as per the experience of your recent visit at AutoZone to complete the survey. After that, provide your personal information to enter the Sweepstakes.


You can also complete the survey by accessing the telephone Survey. Just Dial 1-800-598-8943 and provide the reference number printed on your register receipt and complete the telephone Survey by submitting your answer to the Survey questions. When you have completed the Survey, state clearly the following: your telephone number, name, address, city, state and zip code, as well as comments then press the # button on your telephone handset to receive one entry into the Sweepstakes.

If you do not made purchase at AutoZone and want to enter the sweepstakes, then you can participate by mail. Write your name, address, city, state and zip code, telephone number, date of birth and email address on a plain 3” x 5” card and mail it to: “AutoZone CS Winter 15 Sweepstakes”, PO Box 10879, Rochester, New York 14610.
