After responding to the question, complete the entry form by providing required information such as your name, email address, address, city, state, zip code, phone number, and submit it during promotion period to get one entry into the sweepstakes.

One Grand prize winner and five First prize winners will be selected on or about November 20, 2012 in a random drawing from among all eligible entries during the promotion period. Winners will be informed by overnight mail within 7 business days of selection process.
