• Mail-In Alternative Entry Method: May hand print the eligible entrant’s name, address, city, state, ZIP Code, telephone number, e-mail address, and date of birth on a 3” x 5” card and mail the card in a hand-addressed, stamped envelope to 2022 Debit Card Sweepstakes Program, BANK INDEPENDENT, P.
