There are three ways to get entry in to the sweepstakes

Through survey: first make a purchase from Wal-Mart store, then visit www.survey.walmart.com and enter unique code printed on your cash register receipt and then complete and submit the survey. As soon as you submit your survey you will automatically get one entry in to the sweepstakes.

If you want to get entry without filling survey, then visit www.entry.survey.walmart.com and submit entry form with all required detail like name, date of birth, address, phone number, etc.

Entrant can also get entry in to the survey sweepstakes by mailing their name, mailing address, phone number and age to 2014 Q2 Wal-Mart Sweepstakes, PO Box 6246, Clear Lake, MN 55319.

At the end of the sweepstakes, five winners will be selected randomly and awarded $1000 Wal-Mart gift card.
