In-Person Sweepstakes: Entrants over the age of 18 as of the start of the Promotion Period may enter by visiting a participating Verizon store, scanning a QR code with their mobile device, and completing the online entry form at web.witcontests.com/panthers/sweepstakes/win/ultimate-super-bowl-experience-sweeps-in-store-241216.

Mail-In Entry: To enter without visiting a Verizon store, handprint your first and last name, complete mailing address, phone number, date of birth, and email address on a 3" x 5" index card or piece of paper.
