2. Fill out the required entry form with your information.
3. Upload a photo of your pet (or you and your pet).
4. Confirm and submit your entry.

Pet Insurance Super Bowl LX Ticket Giveaway Winner Selection:

Each week, one winner will be selected from all eligible entries received during that week.

One winner will be selected by a panel of judges on the basis of following judging criteria at the end of the Promotion Period from all eligible entries received.

Entries will be evaluated based on the following criteria:

1. Creativity and/or Originality – 25%
2. Relevance to Theme – 25%
3. Photo Quality – 25%
4. Emotional Impact – 25%
