There are 2 ways to enter:

(A) Moe’s App Method of Entry with Purchase

• You must have the Moe’s Loyalty App, if don’t then you download it to enter. Once you have an account, log in to your account on the Moe’s Loyalty App and follow the links and instructions to make a purchase including any beverage through the App. Then, follow the links and instructions to complete and submit the entry form including your name, a valid email address, and your date of birth and you will receive 1 entry.
• Alternately, Moe’s guests can order (any size drink) online or in the restaurant to qualify for purchases as long as they are signed into their Moe’s Reward account or scan their QR code in the app, at checkout in the restaurant.

(B) Mail-In Alternate Means of Entry: Hand-print your full name, full mailing address, email address, phone number, and birth date on a plain 3x5 inch piece of paper.
