A handprint in English in black ink on a 3”x 5” piece of paper your complete name, mailing address, mobile phone number, email address, and date of birth and then mail it in a properly stamped envelope to “Golden Corral Holiday Sweepstakes, PO Box 488, Syosset, NY 11791-0488”.
