How to Enter

Treatment Entry:

Step 1: You must first register for the Sweepstakes online between 12:00:01 AM ET on Friday, April 15, 2022, and 11:59:59 PM ET on Friday, April 29, 2022. Visit botoxcosmetic20thcelebration.com and follow the instructions to complete and submit the sweepstakes registration form.

Step 2: Purchase and receive a cash-pay BOTOX® Cosmetic (onabotulinumtoxinA) treatment*, in accordance with BOTOX® Cosmetic’s applicable indication, prescribing information or instructions for use from a qualified specialist that participates in Allē and ensures your Provider records your Treatment in your Allē account.

Mail-in Entry. If you are not eligible for or do not qualify for a BOTOX® Cosmetic treatment or you would like to enter without purchasing a BOTOX® Cosmetic treatment, you may enter by mail.

Step 1: You must first register for the Sweepstakes online between 12:00:01 AM ET on Friday, April 15, 2022, and 11:59:59 PM ET on Friday, April 29, 2022. During the Sweepstakes Registration Period, register for the Sweepstakes by visiting botoxcosmetic20thcelebration.com and following the instructions to complete and submit the Sweepstakes Registration.

Step 2: Mail a postcard, including your name, complete address, the email address associated with your Allē account, and phone number associated with your Allē account, to the following address: The BOTOX® Cosmetic 20th Celebration Sweepstakes, PO Box 776, Social Circle, GA 30025-776.

Winner Selection:

The winners will be selected on or about, June 9, 2022, from all the eligible entries received by a random drawing.

Prizes:

20 Grand Prizes will receive a trip package for 2 people, Winner, and 1 guest, to attend a show at The Theatre located at Resorts World Las Vegas in Las Vegas, Nevada. Each Grand Prize trip package includes:

• Round-trip air transportation for 2 to Las Vegas, Nevada;
• 2 nights hotel (single room/double occupancy);
• Round-trip car transportation;
• 2 tickets for a show at The Theatre located at Resorts World Las Vegas; and
• $250 Food and Beverage credit with Resorts World Las Vegas.

The ARV of each Grand Prize is $3,698.

The total ARV of all prizes available to be won is $73,960.
