Step Two: Purchase and receive a cash-pay BOTOX Cosmetic treatment from a qualified specialist participating in Allē during November 1, 2023, and November 30, 2023. Make sure your Provider records your Treatment in your Allē account to receive one entry (Treatment Entry).

Method 2: Free, Alternate Method of Entry (AMOE) / Mail-in Entry

Step One: Register for the Sweepstakes between November 1, 2023, and November 22, 2023, by visiting alle.com/botoxcosmeticday and following the instructions to complete the Sweepstakes Registration.

Step Two: Hand-print your name, address, email address associated with your Allē account, and phone number associated with your Allē account on a sheet of paper. Mail it in an addressed envelope with proper postage to: The BOTOX Cosmetic Day Sweepstakes, PO Box 776, Social Circle, GA 30025-776.
