• The first valid Website submission earns 10 Sweepstakes entries.
• After the initial submission, an entrant may submit the form once per calendar day and receive one additional entry for each valid daily submission.

After entering through the Website, entrants may obtain additional entries by visiting https://www.airmedcarenetwork.com/apply and purchasing or renewing an eligible membership plan.

• AirMedCare Network Membership: up to 10 additional entries.
• Fly-U-Home Membership: up to 20 additional entries.
• Fly-U-Home International Membership: up to 40 additional entries.
• Cancellation of a membership before the end of the Sweepstakes Period invalidates entries awarded from that membership.

Random Drawing

One potential winner will be selected randomly on or around 11/02/2026 at 1:00 p.m. CST from all eligible entries received.

The potential winner will be notified on or around 11/04/2026 at 1:00 p.m. CST by mobile phone or email and must claim the Prize within five calendar days of notification. The required Affidavit of Eligibility and Liability/Publicity Release must also be returned within five days of written notification.
