After that, follow the on-screen instructions to create a ballot by selecting between one (1) and twelve (12) NHL® players (maximum of eight (8) skaters, four (4) goalies; maximum of three (3) players (two (2) skaters and one (1) goalie) (per each division: Atlantic, Metropolitan, Central, or Pacific). As soon as you did it, you will receive the entry form. Just complete it with all the requested details and submit it to receive an entry into the sweepstakes.
