There are 4 ways to enter:

• Purchase Method of Entry: An eligible entrant may visit a participating Darden restaurant location and complete a purchase. Upon making a purchase, the entrant will receive a receipt with a survey code and promotion URL. Entrants may visit the URL at lhtogosurvey.com to complete and submit a survey. Entrant must then complete and submit the registration form by providing the information requested by Sponsor including, but not limited to: complete first and last name, street address (P.O. Boxes are not permitted), city, state/province/territory, ZIP/Postal Code, daytime phone number, date of birth, survey code, and agree to the Official Rules.
• Free Mail-in Method of Entry: An entrant may handwrite his/her complete name (no initials), valid e-mail address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY), on a 3½"x5" card and mail it (in an outer mailing envelope) with sufficient postage to: Guest Satisfaction Survey Sweepstakes, PO Box 211, Macedon, NY 14502-0211. Mail-In Entry is worth 1 entry.
• Scan QR Code: An eligible entrant may visit a participating Darden restaurant location and scan the Promotion QR code provided on the Promotional material. Upon scanning the QR code, the entrant will be prompted to register for the 3 Promotion, provide the survey code, and complete the survey. Upon completing the survey, the entrant will receive 1 entry into the applicable Entry Period.
• Email Invitation: An eligible entrant who receives an email invitation may follow the links and instructions in the email to register and complete the survey. Upon completing the survey, the entrant will receive 1 entry into the applicable Entry Period.
