To take the survey, you just need three things: time, an internet connection, and a laptop or a phone. Receipt Invite Entry
1. Visit https://survey3.medallia.com/StaplesCares and click on the “Begin Survey” button.
2. After that, select the “Survey Code” option to enter the 19-digit survey code from your receipt. If you don’t have a receipt or it’s old, choose the “My survey code is expired/invalid” option.
3. Then, rate your overall experience at Staples and provide genuine feedback for the given score
4. Now, answer the survey questions and rate some services based on your recent shopping experience at Staples
5. If you are 21 years or above then, you can enter the sweepstakes to win a $500 Staples gift card
6. Lastly, provide your personal information for the $500 gift card sweepstakes
Email Invite Entry:
If you have received an email survey invitation, click on the link to access the survey page, and complete the survey by answering all the questions. At the end of the survey, complete the entry form to receive one Sweepstakes entry.
Mail-in Entry:
To enter without making a purchase at a Staples store or without completing the survey, handprint your name, complete address, city, state, zip code, daytime phone number, and birth date on a 3” x 5” card and mail it to: Staples Customer Satisfaction Survey 2021/2022 $500 Monthly Sweepstakes Entry, PO Box 728, Social Circle, GA 30025-728.
Entering the survey and the sweepstakes online, email or mail method does not affect your chances of winning.
Staples Store Survey Sweepstakes Winner Selection
At the end of each monthly entry period, four winners will be selected via random drawing from all the eligible entries received within the corresponding Monthly Entry Period. The drawing dates are 03/10/21, 04/14/21, 05/12/21, 06/09/21, 07/14/21, 08/11/21, 09/08/21, 10/13/21, 11/10/21, 12/08/21, 01/12/22, and 02/09/22. The winner will be notified by email or phone and he/she must confirm the mailing address within 5 business days to qualify for the prize. If not done, an alternate winner will be selected.

74 comments postedToday at Staples in the Rim, San Antonio, TX I returned an item purchased by mistake. I was helped by Kenya. It was a most pleasant experience. Kenya was professional, courteous and efficient. I was impressed and will return to Staples for more merchandise in the future.
I went to the Staples store in Monona, WI and was assisted by Katherine. I brought a puzzle in to
have it laminated. She explained that the puzzle could not be laminated unless it was mounted on
something, so she found some card stock and helped me glue it on and then put it into the machine
and gave me the finished product. She was totally kind and gracious about the entire situation.
The cashier was also very polite, and helped me sign up for a rewards program. I will definitely
return to that store in the future.

Laurie works in the Staples store on Route 70 in Brick, NJ. She assisted me in finding printer ink and made calculations relating to a $36.00 reward I received by e-mail. Unfortunately I could not make a connection to the Staples survey so would appreciate your giving Laurie my thanks for her help and hope she will be given a bonus as she is an asset to your store.
I was at the Roseville Standford Ranch Staples to have something laminated; when I walked up to the counter, the associate Kristin was helping an elderly gentleman who could barely hear and talk. She was speaking loud enough for him to hear, was asking clarifying questions, and was helping him with his cell phone to find the photos he requested (she even typed out a message for him to the person he was sending the photos to). He had brought his personal laptop to download a copy of the photos to, and she showed him exactly where he could find them. She was so respectful and listened to him thoroughly; when he offered to pay her for her extra help, she politely refused and said that it was her pleasure helping him out. It was a wonderful exchange to watch - Kristin deserves a raise!
I was in the store at the Cressona mall in Pa this morning. I needed help printing some material from my computer. I met with Brittanie who was outstanding to deal with. Friendly, quick, and knew what she was doing. A very pleasant experience.

A shout out to JOSE in the Technology/Electronics Department of our Walla Walla, Washington Staples store!! He initially answered some questions over the telephone that I had about a computer monitor and offered to research Dell products to see if the item was available and in stock locally. JOSE was the technician who then assisted me in person at the store. He was very professional, patient and personable! I appreciated Jose's knowledge and his guidance in comparing similar items. I NEEDED INK FOR MY NEW EPSON PRINTER WHICH USED DIFFERENT INK FROM MY OLD DEAD PRINTER. TREY GAVE SO MUCH INFORMATION ABOUT INK AND I HAD A GREAT EXPERIENCE WITH THIS PURCHASE. I WILL CERTAINLY BE GOING BACK TO THE ANKENY, IOWA STAPLES WHEN I NEED INK THE NEXT TIME.
Nathan and Bryan at my Staples store in San Luis Obispo are to be commended! I bought a new Lenovo laptop, but brought in my old Lenovo laptop (that had a faulty computer power connection) for a full data transfer. I know if I would try it myself, I would be severely frustrated as a Senior non-tech person. They patiently transferred all the data, kept me notified as to how it was going, and went through the steps with me on my new computer the day I picked it up.
