You have two alternative ways to take part into this survey sweepstakes:

A) Receipt Invite: In this way, you need a specially-marked cash register receipt that you can get on your purchase at a participating Jo-Ann Fabric and Craft Stores. So, first get that receipt and if you already have it then keep it on hand, go online to access survey website www.telljoann.com. Then you will be required to complete the customer satisfaction survey by answering the survey questions regarding your experience at Joann. At the completion of the survey, you will be invited to fill out the entry form entirety and submit it to receive one promotion entry for that month.

B) Email Invite: As survey invitation, Jo-Ann Fabric and Craft Stores will send you an email. And if you receive that invitation email, then follow the link and instructions of email and take survey. There you will be asked questions regarding your experience at Joann and some of your contact details to receive one promotion entry for the month.

You can also use the alternate means of entry in which you have to simply hand print your name, complete address, city, state, zip code, daytime phone number, email address, and birth date on a 3” x 5” card and mail it to: Jo-Ann Fabric and Craft Stores Guest Feedback Sweepstakes, PO Box 16500, Rochester, NY 14616.
