Visit www.omgaonline.com/lifedreams, follow the instructions, and complete the entry form by providing required information such as name, address, city, state, zip code, email address, and most importantly describe about your life dream, then submit it during promotion period to get one entry into the sweepstakes.

One winner will be selected on or about October 17, 2012 in a random drawing from among all eligible entries received during the entry period. Winner will be informed on or about October 17, 2012 by phone, e-mail or postal mail.

Winner will receive three days, two nights trip for two to a destination of Winner’s choosing within the Continental United States (excluding Hawaii and Alaska). Trip includes airfare and hotel accommodation (ARV: $4,000).
