So, enter, win and escape from your busy day-to-day lifestyle and enjoy the cold weather as well play Golf as at Pinehurst. Back9Network is also giving fifty lucky people a chance to win Back9Network hats!

Limit: There is a limit of one Website entry per Entrant. The age of the entrant must be at least eighteen years of age or older at the time of entry.

There are three ways to receive entries into the Sweepstakes:

1) Website: Visit www.onceinalifestyle.com during the Sweepstakes Period and follow the link to complete and submit the official entry form by providing the required information such as address, city, state, zip, and cable provider and click the “Continue” button. Upon clicking the “Continue” button, you will receive one entry into the Sweepstakes.

2) Refer A Friend:

After completing the form and clicking the “Continue” button, you will get an option to share the Sweepstakes with friends via Email, Facebook or Twitter. You will receive one entry into the Sweepstakes when your friend has entered the Sweepstakes.

i) Email: To invite via Email, when prompted, type your friend’s email address in the field and click the “Send” button to submit the referral(s).
ii) Facebook: To invite via Facebook, select the friends you would like to invite and click “Send” to submit the referral(s).
iii) Twitter: To invite via Twitter, select the followers you would like to invite and then click “Send” to submit the referral(s).

OR

To receive additional entries without using the Refer A Friend via Website entry method, you may enter by submitting a 3x5 card to include full name, physical address, city, state, zip code, email address and mail the completed 3x5 card to: “Once in a Lifestyle” Sweepstakes, Back9Network Inc., One American Row, 10th Floor, Hartford, CT 06103.

3) Cable Provider: You can submit your phone number and click the “Call Me Now” button. Upon clicking the “Call Me Now” button, you will receive a phone call to be connected to your current cable provider. You will need to complete the call by stating that you would like Back9 Network as a channel option through your cable provider. Once the call has been completed, you will receive ten entries into the Sweepstakes. There is no limit to the number of calls and Entrant can make to their cable provider.

On or about August 4, 2014, potential prize winners will be selected in a random drawing.

Prizes:

One Grand Prize: A grand prize winner will be awarded a trip for four to a golf course destination determined by public voting prior to the start of the Sweepstakes. The Grand Prize includes: a $1,000 airfare travel voucher for four, two night hotel accommodations, and two rounds of golf at a golf course destination that was determined by public vote prior to the start of the Sweepstakes. Total ARV of the Grand Prize is $12,000.

Fifty First Prizes: Each First Prize Winner will be awarded a Back9Network-branded hat. Each First Prize has an ARV of $8. Total ARV of all First Prizes is $400.
