Online: After getting an invitation card, visit myphvisit.com and select preferred language and enter receipt/certificate identifier found on your Invitation and then give answer of questions regarding your shopping experience to complete the survey. As soon as you complete the survey, you will get 10 chances to win sweepstakes prize. You can also complete survey by calling or messaging at toll free number written on invitation card.

Application: Entrants are also able to get entry in to the sweepstakes through Sponsor's GoRecommend application. For this, you must be invited by participating client of the Sponsor, either directly or at the end of the client's survey during the relevant Daily Sweepstakes Period.
